Stepping On

A 7-session (2 hours each) falls prevention program proven to decrease falls by 30%.

The program is for people who:

  • Are age 60 or older
  • Live in their own home or independent apartment
  • Are able to walk without the help of another person
  • Do not use a walker, scooter or wheelchair most of the time indoors
  • Do not have dementia

Healthy Living with Diabetes

A 6-session (2.5 hours each) self-management program proven to reduce emergency department visits by 53%.

The program is for adults who:

  • Have, or live with someone who has, diabetes (Type 1, Type 2, or Pre)
  • Do not have dementia

Vivir Saludable con Diabetes

A 6-session (2.5 hours each) self-management program proven to reduce emergency department visits by 53%.

The program is for adults who:

  • Have, or live with someone who has, diabetes (Type 1, Type 2, or Pre)

A 6-session (2.5 hours each) program to help people better manage their chronic pain.

The program is for adults who:

  • Are 18 or older
  • Have, or live with someone who has, chronic pain

Living Well with Chronic Conditions

A 6-session (2.5 hours each) self-management program proven to reduce emergency department visits by 28%.

The program is for adults who:

  • Have, or live with someone who has, one or more chronic conditions
  • Do not have dementia

Diabetes Prevention Program

An evidence-based, year-long behavior change program for people with pre-diabetes.

The program is for adults who:

  • Are 18 years of age or older
  • Are overweight (BMI >24, or >22 if you are Asian American)

AND, one of the below:

  • Have received a medical diagnosis of prediabetes within the past 12 months
  • Have a history of gestational diabetes or given birth to a baby weighing 9 pounds or more

Better Choices, Better Health

An online self-management program for adults with one or more chronic conditions. Log on 2-3 times each week (for a total of 1-2 hours per week) for 6 consecutive weeks.


Powerful Tools for Caregivers

A 6-session evidence-based program for family caregivers to improve self-efficacy, emotional well-being, self-care behaviors, and awareness and use of community resources.


Powerful Tools for Caregivers for Parents of Children
with Special Needs

A 6-session evidence-based program for the parents of children with special needs to improve self-efficacy, emotional well-being, self-care behaviors, and increased awareness and use of community resources.


Walk With Ease

An 18-session program proven to reduce the pain of arthritis, improve overall health, and/or make exercise a regular habit.


Clinical Diabetes Self-Management Education and Support (DSMES)

A patient-guided clinical intervention - provided by an American Diabetes Association Recognized Education Program or an American Association of Diabetes Educators Accredited Program - that provides people with diabetes the skills and tools to better manage their condition.

The program is for adults who have a diagnosis of type 1 or type 2 diabetes and are at one of four critical time points:

  • At diagnosis
  • Annually for assessment of education, nutrition, and emotional needs
  • When new complicating factors (health conditions, physical limitations, emotional factors, or basic living needs) arise that influence self-management
  • When transitions in care occur
