Clinical Diabetes Self-Management Education and Support (DSMES)
A patient-guided clinical intervention - provided by an American Diabetes Association Recognized Education Program or an American Association of Diabetes Educators Accredited Program - that provides people with diabetes the skills and tools to better manage their condition.
The program is for adults who have a diagnosis of type 1 or type 2 diabetes and are at one of four critical time points:
- At diagnosis
- Annually for assessment of education, nutrition, and emotional needs
- When new complicating factors (health conditions, physical limitations, emotional factors, or basic living needs) arise that influence self-management
- When transitions in care occur
